A Maryland driver's license is required to apply for assistance. If you are eligible for SSI (Supplemental Security Income), our office will mail you a copy of the Qualifying Document (birth certificate) with your application. If you are applying for assistance under our Protection and Advocacy Program, you will have to present a copy of the document.
You will probably need to provide your name, address and Social Security number (or, if you are a minor, your date of birth, your mother's name, Social Security number and her date of birth). You may be asked for your Maryland driver's license number as well if you live in New York City.
Do not give away your personal information. The Department of Human Services will not collect it. You must complete an application form and provide your personal information on your own or that of someone else if you qualify to do so. You may also need to answer a series of questions to find out if you are eligible for benefit programs.
Never sign your Social Security number at the Department of Human Services when applying for public assistance because you might be giving away your personal information. This can be a costly mistake because you may need fingerprinting for public assistance...and since Social Security numbers are used for so many things, it can render you stranded.
Protect your identity, make sure to apply in person at the Department of Human Services (or call at 410-260-2055), not at a retail store when the application package is on display. Many retailers take advantage of this program providing the application package from time to time without asking for the person's name or suggesting that you apply over the counter. Some retailers even charge a fee to process your application. Do not let this happen to you or anyone else. The application package is for people seeking public assistance, not the retailer! 7211a4ac4a
